Ở thể cấp tính, heo phát bệnh nhanh, gồm nhiều triệu chứng: Heo thường có biểu hiện sốt cao trên hoặc bằng 410C, nằm li bì, khó thở, thở dốc, ngồi thở giống chó… dễ nhận biết hơn đó là kém ăn hoặc bỏ ăn hoàn toàn. Vùng hầu và vùng mặt có biểu hiện sưng phù, tai và vùng bụng xuất hiện nhiều dấu vết tím đỏ, niêm mạc mắt sẽ bị tím tái và sẽ bị chảy nước mũi (lúc đầu nước mũi có màu đục và sau đó có kèm thêm máu).

Bệnh tụ huyết trùng ở heo thể cấp tính tiến triển rất nhanh từ 1 – 2 ngày. Heo chết rất nhanh, tỷ lệ chết cao. Nếu không chết có thể chuyển sang thể mãn tính của tụ huyết trùng.
